Job summary

Monarch Healthcare, a North Carolina and Rhode Island provider, seeks a Qualified Professional to conduct assessments and develop person-centered plans for individuals with intellectual and developmental disabilities and mental health needs.

The role requires a Bachelor or Master in Human Services with relevant experience and a valid US driver’s license, offering meaningful growth and a comprehensive benefits package.

Qualifications

• Experience working directly with individuals with intellectual and developmental disabilities in residential, community, or day program settings.
• Experience working with individuals with mental health diagnoses or developmental disabilities.
• 2 years of management experience; 4+ years preferred in leadership roles.

Responsibilities

• Provide initial and ongoing assessment and develop person-centered plans; participate in team meetings and coordinate with supports.
• Ensure services align with the person-centered plan, are fiscally viable, and obtain necessary authorizations and billing.
• Train staff on plans and related programs; supervise day-to-day activities and conduct unannounced visits.
• Maintain required reports and documentation in the EHR/medical records; ensure timely and compliant record-keeping.
• Use effective communication to build relationships with individuals, families, staff and stakeholders.
• Assist with hiring, discipline, and performance processes; coordinate employee development per policy.
• Respond to incidents following agency policies; maintain a safe working environment.
• Schedule on-call coverage and comply with all policies and state/federal regulations.
