POSITION SUMMARY:

The Program Manager, Health Justice Mobile is responsible for the day-to-day operational leadership, coordination, and oversight of Community Healthcare Network's (CHN) Health Justice Mobile Program. The program delivers comprehensive healthcare services to individuals transitioning from the criminal legal system, with a focus on patients leaving arraignment court in Brooklyn and experiencing barriers to accessing care.

The Program Manager oversees mobile unit operations, staff, program implementation, patient engagement, community partnerships, data and reporting activities, and compliance with regulatory, contractual, and agency requirements. Serving as CHN's primary liaison with the Fortune Society and other community stakeholders, the Program Manager ensures continuity of care, linkage to services, and achievement of program goals through a patient-centered, trauma-informed, culturally responsive approach. Assigned days and shifts are determined in advance; the Program Manager is expected to maintain flexibility to support the unit's operating calendar.

This position is funded through the Brooklyn District Attorney’s Office and continued employment with CHN is contingent upon the availability of funds.

DUTIES AND RESPONSIBILITIES
  • Provide overall leadership and operational management of the Health Justice Mobile Program.
  • Under the supervision of the VP ID/ LGBTQ Programs work collaboratively with Facilities, Operations and Medical Affairs leadership to develop and implement program work plans, timelines, and deliverables in accordance with contract specifications and funding requirements.
  • Develop, implement, and monitor program policies and procedures to ensure consistency with agency standards, regulatory requirements, patient confidentiality protocols, and best practices for the services delivered on the mobile unit, including primary care, HIV testing and treatment, PrEP/PEP, STI testing and treatment, mental health services, pregnancy testing and counseling, birth control services, emergency contraception, and sexual health education.
  • Oversee day-to-day mobile clinic operations, including patient registration, eligibility verification, scheduling, patient flow, and service coordination.
  • Supervise Health Justice Mobile Unit staff and support performance management, training, and professional development.
  • Serve as CHN’s primary liaison to the Fortune Society and other community partners, coordinating services and resolving operational issues.
  • Oversee referral, linkage, and follow-up processes to ensure continuity of care and successful connection to CHN and community-based services.
  • Plan and oversee outreach, health education, and community engagement activities at mobile unit locations.
  • Collaborate with providers, nurses, patient navigators, and interdisciplinary team members to ensure integrated service delivery.
  • Oversee data collection, documentation, reporting, and quality assurance activities; analyze program data to support continuous quality improvement.
  • Ensure operational readiness of the mobile unit, including supplies, maintenance coordination, safety inspections, and inventory management.
  • Prepare and submit required reports, performance metrics, grant deliverables, and program evaluations.
  • Comply with agency standards for professional conduct, ethics, safety, and service delivery.
  • Perform other related duties as assigned.
EDUCATION AND EXPERIENCE REQUIRED
  • Bachelor's degree in public health, health administration, business administration, or a related field.
  • Minimum of four (4) years of program management experience required, with preference given to health and/or community-based settings.
  • Experience serving underserved, justice-involved, or vulnerable populations preferred.
