Job summary

Community Healthcare Network (CHN) seeks a Program Manager for its Health Justice Mobile program in New York City. This role leads daily operations, coordinates with partners, and ensures high-quality care for individuals transitioning from the criminal legal system.

The manager will supervise staff, manage data/reporting, and ensure compliance with funding and regulatory requirements. Flexibility to support calendar and outreach is essential.

Qualifications

• Bachelor's degree required in a related field.
• Minimum of four years of program management experience required.
• Experience serving underserved, justice-involved, or vulnerable populations preferred.

Responsibilities

• Provide overall leadership and operational management of the Health Justice Mobile Program.
• Collaborate with VP ID/LGBTQ Programs leadership to develop and implement program work plans, timelines, and deliverables.
• Develop, implement, and monitor policies and procedures to ensure compliance and confidentiality.
• Oversee mobile unit operations, including registration, eligibility verification, scheduling, patient flow and service coordination.
• Supervise Health Justice Mobile Unit staff and support professional development.
• Serve as CHN’s liaison to Fortune Society and community partners to coordinate services.
• Oversee referral, linkage, and follow-up processes to ensure continuity of care.
• Plan outreach, health education, and community engagement activities at mobile locations.
• Collaborate with providers and team to ensure integrated service delivery.
• Oversee data collection, reporting, and quality assurance activities for continuous improvement.
• Ensure readiness of the mobile unit, including supplies and maintenance.
• Prepare and submit required reports, grant deliverables, and program evaluations.
• Comply with agency standards for conduct, safety and service delivery.
• Perform other related duties as assigned.

Education

Bachelor's degree in public health, health administration, business administration, or a related field
