Job summary

Community Healthcare Network in New York is seeking an Outreach and Engagement Specialist to support the Care Management team. You will perform patient intake, conduct outreach with community organizations, enroll referrals, and link patients to care, education, and resources.

You will document encounters in charts, handle administrative duties like scanning, and coordinate care with CHN providers. The role emphasizes patient-centered outreach across community sites and supports vulnerable

Qualifications

  • Associates Degree or equivalent of college credits from an accredited college/university required with one year of community outreach or care coordination experience.
  • High School Diploma or GED along with 3 years of community outreach or care coordination experience.

Responsibilities

  • Conduct new patient’s assessment screenings consistent with the Scope of Services.
  • Conduct outreach activities to newly assigned patients both in person and telephonically.
  • Engage in outreach efforts to support patient retention in HH care coordination services and primary care both in person and telephonically.
  • Conduct community outreach visits to patients including to patients homes, shelters, hospitals and other points of contact within the community.
  • Provide expedited visit to patients for urgent situations such as hospitalization.
  • Inform patients of our ancillary services and give them health education materials.
  • Actively seek out and engage in relationship building with Community Based Organizations (CBO) in order to generate referrals to care coordination and other CHN services.
  • Establish working relationship with CHN interdisciplinary team to generate and respond to internal referrals including but not limited to attending care conferences, searching via ECW, facilitating presentations amongst others.
  • Notify Care Team of outcome of assigned care coordination tasks.
  • Assist the patient in selecting a Primary Care Provider (PCP) or Mental Health Provider and offer assistance in arranging an initial visit with their healthcare provider
  • Provide the patient with the Health Home emergency number
  • Educate the patient on the policies concerning covered services and what to do in an emergency
  • Provide the patient with a list of Health Home providers including those based at CHN
  • Inform the new patient about their rights for continuation of an ongoing course of treatment
  • Provide patients with information on managed care organizations
  • Inform patient on their right to “free access” in receiving family planning services
  • Provide patient with general information on chronic conditions, mental illness, HIV prevention and primary care

POSITION SUMMARY:

The Outreach and Engagement Specialist is an integral part of the Care Management team. Some of the primary responsibilities include to patient finding through engagement with community based organizations, timely follow up with referrals, outreach, enroll and complete intake screening for patients assigned, link patients to care, educate patients on their health conditions, educate patients on resources and services for which they are eligible, assist patients with immediate and urgent needs identified during intake, document all patient encounters and interventions in the patient’s charts, perform office administrative duties including the scanning and uploading of documents and delivery of direct care coordination services to patients.
