Job summary

Community Healthcare Network seeks a Program Manager, Health Justice Mobile to lead the day-to-day operations of the health access program serving individuals transitioning from the criminal legal system in Brooklyn and NYC. You will coordinate mobile unit services, staff, and partnerships to ensure trauma-informed, patient-centered care.

The role requires a bachelor’s degree in a related field and at least four years of program management experience, with preference for health/community

Qualifications

• 4+ years of program management experience in health/community settings.
• Experience serving underserved or justice-involved populations preferred.

Responsibilities

• Provide leadership and operational management of the Health Justice Mobile Program.
• Collaborate with Facilities, Operations and Medical Affairs to develop and implement work plans and deliverables.
• Oversee mobile unit operations, patient registration, scheduling, and service coordination.
• Supervise staff and support training and professional development.
• Liaise with community partners (e.g., Fortune Society) to coordinate services and ensure continuity of care.
• Oversee data collection, reporting and quality assurance to support continuous improvement.
