Community Healthcare Network (CHN) in New York, NY, is seeking a Program Manager for the Health Justice Mobile program. You will lead day-to-day operations, coordinate care for justice-involved individuals, and work with partners to ensure continuity of services.

The role focuses on delivering primary care, HIV testing/treatment, STI testing and education, mental health services, and reproductive health in a mobile setting, with data-driven quality improvement and regulatory compliance.

Qualifications

• Bachelor's degree in public health, health administration, business administration, or related field.
• Minimum of four (4) years of program management experience required, with preference given to health and/or community-based settings.
• Experience serving underserved, justice-involved, or vulnerable populations preferred.

Responsibilities

• Provide overall leadership and operational management of the Health Justice Mobile Program.
• Work with leadership to develop and implement program work plans, timelines, and deliverables in accordance with contract specifications and funding requirements.
• Develop, implement, and monitor program policies and procedures ensuring compliance with standards and confidentiality protocols across services (primary care, HIV testing and treatment, PrEP/PEP, STI testing and treatment, mental health, contraception).
• Oversee mobile unit operations, patient registration, eligibility verification, scheduling, patient flow, and service coordination.
• Supervise Health Justice Mobile Unit staff and support performance management and development.
• Serve as CHN’s liaison to community partners, coordinating services and resolving operational issues.
• Oversee referral, linkage, and follow-up processes for continuity of care.
• Plan outreach, health education, and community engagement activities at mobile unit locations.
• Collaborate with providers and interdisciplinary team for integrated service delivery.
• Oversee data collection, reporting, and quality assurance; analyze data for improvements.
• Ensure readiness of mobile unit: supplies, maintenance, safety, and inventory.
• Prepare reports, metrics, grant deliverables, and program evaluations.
• Ensure compliance with agency standards for conduct and safety.
