SUMMARY AND ESSENTIAL FUNCTIONS:

The Program Manager and Diabetes Nurse Educator (PMDNE) delivers comprehensive and seamless services that bridge the gap and integrate clinical and self-management aspects of diabetes and cardio metabolic care. The Diabetes Nurse Educator is an integral part of the interprofessional team and provides collaborative, comprehensive and person-centered care, and education conducive to behavior change and improved quality of life across the adult lifespan. The Diabetes Nurse Educator supports and advocates for people affected by diabetes to optimize quality care. The Diabetes Nurse Educator promotes self-management to achieve individualized behavioral and treatment goals that reduce risks and optimize health outcomes. The PMDNE aggregates data for the development and implementation of continuous quality improvement (CQI) activities, disseminates research findings to the advisory panel, seeks stakeholder feedback and submits pertinent data to the program accrediting organization, the Association of Diabetes Care and Education Specialists (ADCES) for annual status report requirements to maintain program accreditation. In addition, the PMDNE maintains balance of department budgeting, productivity reporting, and delivery of required quarterly department reports.

Job Responsibilities

  • Works under the direction of the LRH Chief Nursing Officer. Reports to the Senior Director of Nursing Services.
  • Integrates skills and knowledge of pathophysiology, epidemiology, clinical management, cardio metabolic conditions, and self-management of diabetes into clinical practice.
  • Advocates for and communicates about improved quality of care and outcomes for those living with, at risk for, and affected by diabetes and cardio metabolic conditions.
  • Actively participates in the quality improvement (QI) process and adapts practice/process based on QI findings.
  • Reviews patient appointment billing charges and remains current on the appropriate billing codes and associated charges.
  • Manages department budget utilizing organizational budgeting software.
  • Partners with individuals to deliver care and education conducive to behavior change and improved quality of life for self-management of diabetes and cardio metabolic conditions across the lifespan.
  • Contributes to research and applies current research and evidence-based care to practice.
  • Applies business principles, systems practice, and population health management to support achievement of the Quadruple Aim (reduced costs, better outcomes, improved experience, and improved work life for healthcare providers).
  • Applies the ADCES core self-care behaviors (AADE7) to educate on and initiate behavior change.
  • Engages in lifelong learning and serves as a role model of professionalism.
  • Provides quality diabetes self-management education in individual and group settings based on assessed needs. Utilizes appropriate teaching techniques that are sensitive to the learning preferences of the person with prediabetes or diabetes.
  • Follows the ADCES Diabetes Education Accreditation Program (DEAP) requirements which are based on the National Standard of Diabetes Self-Management and Support Guidelines.
  • Performs data aggregation, analysis and dissemination to annual Advisory Panel and ADCES annual status reporting.
  • Completes comprehensive assessments for each patient including emotional and behavioral health, interprets personal health data, develops an individualized care plan based on the patient’ assessed needs and goals and promotes successful self-management.
  • Documents all individual contacts/visits in the Electronic Health Record and outcomes data base according to the guidelines in a timely manner.
  • Collaborates, advocates, and confers other members of the diabetes care team in developing person-centered diabetes plans.
  • Advocates for and supports technology-enabled diabetes education and care in individuals and population health diabetes services.
  • Collaborates with community partners to establish and maintain on-going support options.
  • Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards and safety standards. Complies with governmental and accreditation regulations.
  • Utilizes telehealth as applicable and appropriate to ensure equitable access to healthcare is provided to all patients.
  • Participates in meetings, serves on committees, and represents the department and hospital/facility in community outreach efforts as appropriate.

SUPERVISES: None

Qualifications

  • Must possess Certified Diabetes Care and Education Specialist Certification (CDCES) (preferred) or will sit for board examination within 24 months of hire.
  • If Registered Nurse must be a graduate of an accredited school of nursing, BSN preferred. Active licensure in State of NH.
  • Three years of clinical nursing experience, medical surgical nursing preferred. Minimum of 6 months experience providing diabetes education and/ or 15 contact hours within past year in diabetes clinical and educational specific to care of people with diabetes.
  • Active BLS certification.
  • Must maintain 15 hours of continuing education annually specific to diabetes or diabetes related topics or behavior change.

Experience/Specialized Skills

  • Excellent verbal and written communication skills.
  • Basic computer skills including use of Microsoft Office.
  • Has working knowledge of diabetes technology.
  • Highly organized and strong attention to detail.
  • Strong problem solving and critical thinking skills.
  • Must be able to work with a diverse patient population and have exceptional customer service skills.

Working conditions & Physical environment

  • ADA: Some skills/physical requirements can be altered and appropriate accommodations or use of adaptive equipment will be made in accordance with the ADA. Associates should discuss accommodations and provide medical documentation to Occupational Health.
  • ATTENDANCE: Regular job attendance is an essential job function.
  • Hybrid role not to exceed 50% of time in the remote setting unless granted permission from supervisor.
  • May be required to travel to various locations as needed. Reliable transportation may be required.
  • Working at another site may be required.

DIRECT PATIENT CARE/CONTACT: Exposure to patients, visitors, and family members with a variety of physical and psychiatric/mental health conditions. Visual acuity including color vision. Ability to hear and orally communicate. Ability to read, comprehend, and write English language. Manual dexterity.
