Job summary

LifeStream, Inc. is seeking a Program Manager to lead a team of 14-20 Direct Support professionals and individuals in delivering exceptional care.

You will oversee program operations, ensure supports are delivered, and maintain participant engagement in activities aligned with DHSP goals. Responsibilities include training staff, conducting performance evaluations, and coordinating with Program Director, Residential Clinical staff, and DDS Service Coordinators to ensure resources meet client

Qualifications

• Associates degree in Human Services or equivalent.
• Three years’ experience in day program setting.
• Demonstrated history of quality work performance and teamwork.
• Proven Leadership skills.

Responsibilities

• Ensure all program supports and services are delivered.
• Ensure all program staff maintains expected levels of participation in program operations.
• Provide on the job training and modeling for all employees and direct the activities of employees.
• Conduct Employee Performance Evaluations for program support staff during assigned time frames.
• Communicate effectively with Program Director and Assistant Director, Residential Clinical staff, and DDS Service Coordinators to assure that all support needed are available to each client’s area of interest and/or need.
• Evaluate the unique support needs not included within the DHSP process and utilize all resources to address these needs.
• Ensure daily activity schedule is adhered to and that the chosen activities correspond to DHSP objectives.
• Ensure staff decisions are based upon consumer choice and empowerment.
• Assist individuals in the hiring and evaluation of program staff
• Monitor health care needs of individuals served in coordination with the Health Care Supervisor to assure that delivery of these services occurs in a timely fashion.
• Ensure that support remains flexible and responsive to client needs/interests.
