Job description

CMS launched the Guiding an Improved Dementia Experience (GUIDE) Model to support Medicare beneficiaries with dementia and their unpaid caregivers, particularly those living in underserved communities. The GUIDE Model focuses on providing high-quality, comprehensive, and coordinated dementia care, with the goal of improving quality of life and enabling people with dementia to remain in their homes for longer. Under the model, the Johns Hopkins Care Navigators will assist patients with dementia and their caregivers in accessing both clinical and non-clinical services and support, including respite care. The Administrative Program Manager will play a pivotal role in building and maintaining the Johns Hopkins GUIDE program alongside an interdisciplinary team.

Guiding an Improved Dementia Experience (GUIDE) is seeking a Program Manager who will be responsible for the overall management of the GUIDE Program at JHM. The service is primarily located within the Bayview Medical Center. The Program Manager will be responsible for the development and implementation of processes for the efficient scheduling of patients, management of clinical staff, and clerical functions and workflow, with direct supervision of 2 memory care navigators. Financial responsibilities will include working with GUIDE Administration to develop and implement the Program’s budget.

Oversight of the CMMI requirements, which is the major funding source, and its regulations will be a major job function of this position as well as working with CMS personnel, external vendors, JH IT, JH Billing and Compliance, and GUIDE Administration. The Program Manager will coordinate continuing education to the 2 care navigators. The Program Manager will ensure that the care navigators provide assessment and care coordination of GUIDE patients in adherence to the CMMI Guide Model requirements.

Specific Duties & Responsibilities

  • Responsible for the recruitment, education, training, and supervision of the Care navigators.
  • Actively manages personnel within the service.
  • Assesses staff workload and determines patient assignments.
  • Works in collaboration with the Director to determine individual caseloads.
  • Facilitates effective teamwork between the program and the other disciplines.
  • Conducts performance appraisals for care navigator staff.
  • Responsible for the development and implementation of policy and procedures within the service.
  • Directs weekly meetings with the GUIDE team and monthly meetings with all the providers who see patients in the program.
  • Meets regularly with the Director to discuss availability of resources and prioritization of protocols.
  • Assists care navigators and providers with the completion and submission of patient consent and alignment forms.
  • Maintains regulatory compliance, including oversight of the clinical work within the Program.
  • Assists the Director with GUIDE care navigation protocol development, including reviewing and assessing new protocols for clarity, thoroughness, and logistical feasibility, maintaining clinical excellence.
  • Maintains oversight of compliance with CMMI requirements and any other applicable State and Federal requirements related to the GUIDE model, billing, HR, and institutional policies.
  • Initiates and maintains communication with CMMI to ensure high-quality data submission, timely submission of data, and adherence to guidelines.
  • Collaborates with GUIDE Administration for the development and maintenance of the program's budget.
  • Works closely with the Psychiatry Billing department to ensure we are maximizing revenue capture through accurate billing.
  • Organizes team meetings to evaluate and coordinate care with multidisciplinary team members.
  • Participates in GUIDE and University meetings for continuing staff education.
  • Maintains current knowledge of GUIDE requirements and other applicable regulations.
  • Prepares and maintains program statistics, volume, and financial reports for program leadership, JHU, and CMMI.
  • Participates in development, writing, and preparation of communications, reports, including budgets, timelines, and administrative oversight.
  • Represents the program at CMMI and other GUIDE-related external meetings.
  • Maintains the organization, policies, and procedures for the GUIDE program.
  • Functions as the supervising HRSA CAREWARE (CW) administrator for the PAHAP – responsible for the entry, tracking, and monitoring of all clinical and multiple other mandated data elements into the CW database.
  • Maintains detailed knowledge of CMMI Data Reporting requirements, standards, and performance measures and tracks program performance on these measures.
  • Performs supervisory and training duties of program staff in coordination with the Directors, including participation in recruitment, mentorship for staff development, and performance evaluations of program staff in order to retain a diverse workforce.
  • Administers and ensures compliance with human resources policies and procedures, observes and assesses quality and productivity of work; provides feedback and discipline, as necessary; may provide technical supervision; develops knowledge, skills, and abilities of staff, students, faculty and/or other clients; plans and supports employees in career development opportunities.
  • Will actively work with, maintain relationships with, and coordinate with faculty and staff in the Departments of Psychiatry, Medicine, Neurology, and Nursing.

  • Willingness to work off-site up to 25% of the time. Must have own transportation. Valid Maryland driver’s license.

Minimum Qualifications

  • Bachelor's Degree.
  • Five years related experience.
  • Additional education may substitute for required experience and additional related experience may substitute for required education beyond HS Diploma/Graduation Equivalent, to the extent permitted by the JHU equivalency formula.

Preferred Qualifications

  • Master's Degree in Social Sciences, Public Health, Public Policy, or related field.
  • Demonstrated experience with project design or management in health services, public policy, and/or health organization design or change.
  • Demonstrated experience in supervisory and/or program lead responsibilities.
  • Demonstrated experience with data monitoring and reporting. Familiarity with CMS requirements highly preferred.
  • Experience working with older adults or patients with dementia and their families.
