Qualifications

• Bachelor’s degree plus one year of related experience.
• Valid driver’s license with acceptable driving record.

Responsibilities

• Develop written program plans (IPPs and IPs) for each member, evaluating progress and goals.
• Implement systems supporting goals in social/emotional, vocational, life skills, and community areas.
• Provide instruction in social skills, medication, money and time management.
• Provide individual and family counseling for each member.
• Assist members in developing and navigating natural supports in the community.
• Maintain records and documentation with accuracy and clarity.
• Use proactive approaches and crisis intervention as needed.
• Communicate members’ needs and goals to staff and other parties.
