Vista Life Innovations invites applications for a Program Counselor (Case Manager) to coordinate counseling and case management for Engage members in Madison, CT. You will develop individualized plans and coordinate services, fostering independence and community involvement.

The role requires a Bachelor's degree plus one year of related experience, a valid license, and certifications or the ability to obtain them within 90 days. Strong independence, judgment, and transport duties are essential.

Qualifications

• Bachelor’s degree plus one year of related experience.
• Valid driver’s license and an acceptable driving record.
• CPR, First Aid and PMT certification or ability to obtain within 90 days of hire.
• Ability to work independently, use good judgment, and show initiative.
• Transport students and members to community sites in various settings.

Responsibilities

• Develop written program plans (IPPs and IPs) for each member.
• Implement systems supporting member goals in social/emotional, vocational, life skills, and community involvement.
• Provide direct instruction in areas such as social skills, medication, money management, and time management.
• Provide individual and family counseling for each member.
• Assist members in developing and navigating natural supports.
• Maintain accurate records and documentation.
• Use crisis intervention strategies for challenging situations.
• Communicate members’ needs and goals to staff and other parties.
