Job summary

CrossWinds Counseling & Wellness is seeking a Case Manager to provide quality case management services to adults, helping them adapt to life situations, access resources, and pursue employment while minimizing hospitalizations. The role emphasizes a strength-based approach and collaboration with clients and partners.

The position requires an Associate degree (or higher) with experience in health and human services, plus ability to complete Case Management Training within six months.

Qualifications

• Associate degree plus two years’ experience in health and human services, or four years’ experience in the field.
• Bachelor’s degree in psychology, social work, or counseling is preferred.
• Must complete Case Management Training within 6 months of hire.

Responsibilities

• Provides direct case management services to adults weekly to support community integration.
• Meets or exceeds the billable hour standard for client contact hours.
• Assists clients to identify, secure, and maintain community resources.
• Coordinates referrals and delivery of services among community agencies and within the organization.
• Develops individualized care plans based on a strengths assessment and maintains records.
