Job summary

New Vista's Ethan Crossing location in Springfield is seeking a Case Manager to support clients in behavioral health services.

You will conduct assessments, develop treatment plans, and coordinate care with referral sources, under supervision.

The role requires CDCA certification, high school diploma, and experience in mental health or addiction services; compassionate teamwork encouraged.

Qualifications

• CDCA certification required.
• High school diploma required; 40 hours education in chemical dependency counseling/clinical methods.
• Ability to make safe judgments in client care.
• Social services / human services experience is a plus.
• Experience in mental health and/or addiction preferred.

Responsibilities

• Conducts bio-psychosocial assessments under supervision.
• Provides group counseling and teaches coping mechanisms.
• Identifies issues, creates goals, and develops treatment plans.
• Leads group/individual sessions as necessary and attends treatment teams when required.
• Prepares written reports and case summaries in accordance with program standards and professional ethics.
• Ensures timely documentation meeting facility and regulatory standards.
• Facilitates safe discharge plans, coordinates care with referral sources and community partners.

JOB REQUIREMENTS:

• CDCA certification
• Ability to make safe judgment calls in client care
• Social Service experience is a plus
• Human Services experience is a plus
• Experience in mental health and/or addiction preferred

EDUCATION

High School Diploma, CDCA certification, and 40 hours of education in chemical dependency counseling/clinical methods required .
