Job summary

Key Rehab is seeking a full-time Program Coordinator – OTA or PTA to join our Humboldt Area team in a SNF setting. You will provide therapy within your scope of practice and coordinate program operations, acting as liaison and supporting clinicians while managing daily scheduling.

Preferred candidates have LTC experience, strong organizational and communication skills, and familiar with Medicare billing regulations. Sign-on bonus available up to $2,500.

Qualifications

• Completion of an accredited PTA/OTA program.
• Current PTA/OTA license to practice in assigned state(s).
• CPR certification as required by community.
• Valid driver license and car insurance as required by state law.
• Other items as required by federal, state and local standards.
• Knowledge of Medicare billing regulations.

Responsibilities

• Serve as liaison between Key Rehab and community staff, minimizing administrative burden.
• Coordinate staffing and daily scheduling, support clinicians, and meet KPIs under Regional Supervisor.
