Job summary

Key Rehab in Falls City, NE seeks a full-time Program Coordinator–OTA or PTA to join our SNF team. You will act as liaison between Key Rehab and community staff, supporting clinicians by reducing administrative burden and coordinating daily scheduling.

The ideal candidate has completed an accredited PTA or OTA program, holds an OTA/PTA license in Nebraska, and brings strong organizational and communication skills, plus familiarity with geriatric care and Medicare guidelines.

Qualifications

• Completion of an accredited PTA or OTA program.
• Preferred: prior experience in long-term care and service coordination.
• PTA or OTA license to practice in the state.
• CPR certification as required by the community.
• Valid driver’s license and car insurance as required by state law.
• Knowledge of Medicare billing regulations.

Overview

Key Rehab is looking for a full‑time Program Coordinator–OTA or PTA to join our team in providing excellent care in the Falls City, NE region in a SNF setting.

Sign‑on bonus: up to $2,500.

Education

• Completion of an accredited PTA or OTA program.

Experience

• Preferred: Prior experience in a long‑term care setting and service coordination.

Licenses / Certificates

• PTA or OTA license to practice in the state(s) assigned.
• CPR, as required by the community.
• Valid driver license and car insurance as required by state law.
• Other items as required by federal, state, and local standards.

Preferred Qualifications

In addition to providing therapy under their disciplinary scope of practice (OTA or PTA), the Program Coordinator serves as the liaison between Key Rehab and community staff. This position is instrumental in minimizing administrative burden and providing support to allow clinicians to deliver therapy services. The role coordinates staffing and daily scheduling to meet key performance indicators and expectations under the direction of a Regional Supervisor.
