This role emphasizes compassionate, team-focused service and reliable coordination of daily tasks.

Qualified candidates have a two-year degree in a social services field and at least two years' experience with adults with disabilities or brain injuries in rehab or long-term care.

Qualifications

• Two-year degree in a social services field or equivalent experience.
• Minimum of two years of experience with adults with disabilities and/or traumatic brain injuries in rehab or long-term-care settings.
• Ability to model excellent work behaviors and effective team problem solving.
• Strong problem-solving skills and ability to self-motivate and manage time and priorities.

Responsibilities

• Assist the Program Manager to ensure that tasks on resident care plans are completed.
• Assist with schedule changes.
• Ensure completed documentation for residents and programs.
• Provide quality personal care services and health-related services as needed.
