Job summary

Accessible Space Inc. in New Brighton, MN is seeking a full-time Program Coordinator to assist the Program Manager in delivering resident care plans and coordinating schedules.

You will ensure documentation is completed, provide quality personal and health-related services, and support residents within the community. The role emphasizes teamwork, reliability, and compassionate care in a rehabilitation or long-term-care setting.

Qualifications

• Two-year-degree in social services or equivalent education/experience.
• Minimum two years' experience with adults with physical disabilities or TBI.
• Ability to model teamwork and effective time management.
• Ability to accompany/transport residents within the community.

Responsibilities

• Assist Program Manager to ensure tasks on resident care plans are completed.
• Assist with schedule changes and maintain documentation.
• Provide quality personal care and health-related services.

We are seeking a full time Program Coordinator for our location in New Brighton!

Program Coordinator Responsibilities

• Assist the Program Manager in ensuring that the tasks on resident care plans are completed
• Assist with schedule changes
• Ensure completed documentation
• Provide quality personal care services and health-related services

Program Coordinator Qualifications

• A two-year-degree in a social services area of study or an equivalent combination of experience and education
• Minimum of two years of experience working with adults with physical disabilities and/or traumatic brain injuries in a rehabilitation or transitional long-term-care setting
• Ability to model excellent work behaviors and team-oriented problem solving
• Strong problem-solving skills and the ability to self-motivate and effectively manage time and shift priorities
• Ability to accompany and/or transport residents within the community
