Job summary

Accessible Space Inc. in Coon Rapids, MN seeks a full-time Program Coordinator to support resident care plans and coordination. You will assist the Program Manager, handle schedule changes, ensure documentation, and deliver quality personal and health-related services. Qualifications

• A two-year-degree in a social services area of study or an equivalent combination of experience and education.
• Minimum of two years of experience working with adults with physical disabilities and/or traumatic brain injuries in a rehabilitation or transitional long-term-care setting.
• Ability to model excellent work behaviors and team-oriented problem solving.
• Strong problem-solving skills and the ability to self-motivate and effectively manage time and shift priorities.
• Ability to accompany and/or transport residents within the community.

Responsibilities

• Assist the Program Manager in ensuring that the tasks on resident care plans are completed
• Assist with schedule changes
• Ensure completed documentation
• Provide quality personal care services and health-related services
