Job summary

Accessible Space in New Brighton, MN is seeking a full-time Program Coordinator to support the Program Manager in delivering resident care plans, coordinating schedules, and ensuring complete documentation. This role emphasizes compassionate, team-focused service and reliable coordination of daily tasks.

Qualified candidates have a two-year degree in a social services field and at least two years' experience with adults with disabilities or brain injuries in rehab or long-term care.

Qualifications

• Two-year degree in a social services field or equivalent experience.
• Minimum of two years of experience with adults with disabilities and/or traumatic brain injuries in rehab or long-term-care settings.
• Ability to model excellent work behaviors and effective team problem solving.
• Strong problem-solving skills and ability to self-motivate and manage time and priorities.

We are seeking a full time Program Coordinator for our location in New Brighton!

Program Coordinator responsibilities:

• Assist the Program Manager in ensuring that the tasks on resident care plans are completed
• Assist with schedule changes
• Ensure completed documentation
• Provide quality personal care services and health-related services

Program Coordinator Qualifications:

• A two-year-degree in a social services area of study or an equivalent combination of experience and education
• Minimum of two years of experience working with adults with physical disabilities and/or traumatic brain injuries in a rehabilitation or transitional long-term-care setting
• Ability to model excellent work behaviors and team-oriented problem solving
• Strong problem-solving skills and the ability to self-motivate and effectively manage time and shift priorities
• Ability to accompany and/or transport residents within the community
