Sunrise Community, Inc. in Naples, FL is seeking a Program Coordinator to support individuals with intellectual and developmental disabilities in community living.

The role requires developing and coordinating individualized program plans and training, with strong documentation and team communication. The ideal candidate will have a high school diploma or equivalent (bachelor’s degree preferred), a valid driver’s license with endorsements, and experience in IDD services.

Qualificações

• Must possess a valid driver’s license with endorsements and 3+ years driving experience.
• Advanced computer skills required.
• High School Diploma or equivalent; Bachelor’s degree preferred.
• Experience in IDD field preferred.

Responsabilidades

• Develop, implement and coordinate individual program plans.
• Create, implement and monitor individualized training plans.
• Ensure plans are person-centered reflecting the individual’s hopes and dreams.
• Maintain accurate documentation per regulatory requirements and communicate with team members.
• Communicate with all team members, including family, guardians and support coordinators to ensure that the individual’s needs are being addressed.
