Job summary

Emmaus Community is seeking a Full Time Program Coordinator- Community Services (Non-Exempt) in Pittsburgh, PA. The role mixes direct care/service provision with administrative duties at the Welcome Center, with some evenings and weekends as needed.

Reporting to the Program Manager, the position requires DSP experience and strong communication skills; travel between sites may be required to support individuals and teams.

  • Leadership experience preferred. Good organizational skills with ability to prioritize tasks.

Responsibilities

  • Facilitate services for In-Home and Community Supports, Companion, and CPS.
  • Onboard new individuals and new direct care staff; handle administrative responsibilities.
  • Communicate with ISP team members including family, supports coordinators, and other professionals.
  • Maintain respectful relationships with individuals and follow health/dietary needs.
  • Plan social and recreational activities both at home and in the community.
  • Coordinate service delivery to meet individual needs; assist in staff training on ISPs and related topics.

Salary Range: $39,520.00 - $45,760.00

Title: Full Time Program Coordinator- Community Services (Non-Exempt)

Primary Location: Time will be split between direct care service provision / homes visits (direct service provision) and Welcome Center based on the needs of the program.

Remote Work Position: No. This position does not include a remote work option due to the nature of the responsibilities.

Primary Schedule: Office work / administrative tasks will be completed Monday thru Friday 9a-5p; Direct Care / onboarding hours will be determined by the needs of the individuals. Flexibility required; some evenings / weekends required.

Qualifications

2 years full-time DSP experience

Excellent communication skills (oral and written); working knowledge of the ISP process, incident management, and HCSIS. Ability to use all agency devices and technologies. Leadership experience preferred. Good organizational skills with ability to prioritize tasks. Willing to work with all individuals at all sites as needed.

Driver’s License, Act 34 clearance & FBI clearance

Job Summary

The position of Program Coordinator- Community Services will work with the Program Manager to facilitate services for In-Home and Community Supports, Companion, and Community Participation Supports (CPS). The Program Coordinator will be involved in onboarding new individuals and new direct care staff as well as administrative responsibilities.

Job Description
  • Ability to work as a team member, and agree to supervision by Program Manager
  • Willingness to carry out the mission and support the values of the Emmaus Community
  • Onboard new individuals which includes providing direct care until a DSP is identified
  • Communicates with all ISP team members as needed including family members, Supports Coordinators, behavior specialist, job coaches, day programs, etc.
  • Maintain respectful and meaningful relationships with the individuals
  • Adhere to and assist the individual with following all health and dietary needs
  • Plan and provide individuals with opportunities for socialization and recreation both at home and out in the community
  • Assist individuals with money management, and follows strict policies on protecting and safeguarding client and agency funds
  • Serve as back up DSP when there are call offs
  • Complete compliant service notes and goal documentation
  • Must fulfill the 24 hours of training requirement per year
  • Participates in the development of the ISP, ISP Annual Update & all ISP revisions
  • Reviews ISP, ISP Annual Update and all ISP revisions & attends all applicable meetings
  • Prepare for and attend SIS & IM4Q meetings
  • Acts as Plan Lead for all individuals receiving services through an SCO
  • Reports content discrepancies to the SC and plan team members
  • Ensures that ISP is implemented as written
  • Monitors and evaluates services
  • Completes the quarterly documentation of individuals’ participation and progress toward outcomes
  • Reports changes related to individuals’ needs to the SC and plan team members
  • Reviews the ISP & all 90 Day Reviews with the individual
  • Provides documentation of the plan review to the SC and plan team members
  • Informs plan team members of the option to decline the ISP review documentation
  • Recommends revisions that may involve the deletion, addition or modification of an outcome or service
  • Coordinates the provision of services to meet the needs of the individual
  • Assists in staff training on ISPs, SEE plans, Restrictive Procedures, Behavior Plans, Incident Management, Crisis Intervention and other topics related to programming
  • Oversees newly hired DSP shadowing schedules, coordinating with HR; facilitates smooth transition of DSP scheduling
  • Assists in identifying affordable and creative activities in the community for individuals and communicates with DSPs
  • Reviews completion of compliant service notes
  • Carries out incident management responsibilities including entering reports into EIM, data entry corrective action plans, remediation, etc.
  • Serves as back up Program Specialist for Residential Program Specialist and back up RA as needed
  • "On-call" responsibilities
