Job description

Position: Program Coordinator – Care Parent Network (CPN)

Location: Concord, CA – Providing services Contra Costa County

Status: Full-time, Exempt

VistAbility is looking for a Program Coordinator for our Care Parent Network (CPN). This role supports families of children with intellectual and developmental disabilities (IDD) in the community. The Program Coordinator leads daily work of the CPN, provides guidance to families, supports staff, and strengthens partnerships in the community.

Because this role provides guidance and support to families, lived experience as the primary caregiver of a child with an intellectual or developmental disability (current or past) is required.

Responsibilities:

Family Support & Program Coordination

  • Oversee daily CPN operations, including parent support, information & referral, trainings, and resource navigation.
  • Conduct intakes, needs assessments, and service planning for families.
  • Build ongoing relationships with families, Regional Center case managers, and community partners.
  • Represent CPN at community events, outreach activities, and collaborative meetings.

Program Quality & Compliance

  • Monitor service delivery, family satisfaction, and documentation quality.
  • Recommend and implement program improvements.
  • Identify barriers to access and contribute to program development.
  • Provide orientation, training, guidance, and supervision to assigned staff.
  • Conduct performance evaluations and recommend personnel actions as needed.
  • Support staff development and ensure training requirements are met.

Administrative & Operational Duties

  • Coordinate workshops, special events, and community trainings.
  • Maintain accurate records, documentation, and program reports.
  • Serve as program lead when the Director of CAS is unavailable.
  • Collaboration with Development Team – provide data, stories, and documentation needed for grants and funder reporting.
  • Ensure program activities align with funded goals and deliverables.

Requirements:

  • At least 3 years of experience supporting families of individuals with IDD.
  • At least 1 year of supervising staff or volunteers.
  • Lived experience: Must be the primary caregiver of a child with an IDD (current or past).
  • Strong communication, organization, and teamwork skills.
  • Ability to use email, Microsoft Office, and basic data systems.
  • Valid CA driver’s license, good driving record, and an insured vehicle.
  • Bilingual applicants preferred but not required.

Compensation:

Salary: $72,540 annually

Medical, Dental, Vision, Life Insurance, and LTD insurance valued at approximately $424.84 per pay period ($10,196.16 annually).

Paid time off:

  • 12 sick days per year
  • 10–25 vacation days per year (based on tenure)
  • 30 hours of bereavement leave
  • Up to 2 weeks of paid jury duty

Retirement:

  • 2% employer 403(b) match after one year of service
  • Employer pays 100% of retiree dental and Medicare supplement premiums for eligible retirees.

Employees may be eligible for discretionary bonuses. Mileage is reimbursed at the IRS rate for approved business travel. Required business use of a personal cell phone is reimbursed through a semi monthly stipend.
