Job summary

Damar Services, LLC is seeking a dedicated Program Coordinator to manage referrals, coordinate assessments, and support families within our mission-driven behavioral health programs. You will interact with communities and referral sources to ensure timely, accurate scheduling and documentation.

The role emphasizes collaboration, data accuracy in the EHR, and adherence to training and licensure requirements, with benefits including comprehensive health plans and tuition assistance.

Qualifications

• Minimum of 2 years of case management experience.
• Must be 18 or older and able to pass background checks.
• CPR certification required.
• Valid Indiana driver’s license and driving record acceptable.

Responsibilities

• Attend mandatory staff meetings and other meetings as assigned.
• Receive and track incoming referrals.
• Request and gather pertinent information from referral sources.
• Collaborate with families and team members for efficient scheduling.
• Maintain evaluator schedules to ensure timely appointments.
• Communicate with families and referral sources regarding referral status.
• Manage data in the EHR system.
• Seek efficient use of materials, equipment, and staff to maximize effectiveness.
• Ensure accurate and legible documentation across reports and notes.
• Complete mandatory training and licensure-related items on time.
