Qualifications

• High school diploma or GED required; bachelor's degree preferred.
• Minimum of 2 years of experience in case management.
• Must pass background checks and drug screen.
• Valid Indiana driver’s license and driving record per policy.
• Pre-employment drug screen and physical required.

Responsibilities

• Attends mandatory staff meetings and other meetings as assigned.
• Receives and tracks incoming referrals.
• Requests and gathers pertinent information from referral sources.
• Collaborates with families and team members for efficient scheduling.
• Maintains evaluators schedule to ensure timely scheduling of appointments.
• Communicates with families and referral sources regarding the status of referrals.
• Manages data in the EHR system.
• Seeks the best use of materials, equipment, and staff to maximize efficiency.
• Acts as a good steward of Damar and client finances.
• Takes pride and ownership in Damar’s facilities and assigned work areas.
• Conducts regular file audits to ensure alignment of billable hours, progress notes, invoices, and other reporting.
• Participates in data collection and uses data to improve outcomes and service delivery.
• Maintains documentation that is accurate and legible.
