Qualifications

• 2 years of experience in case management required.
• Strong communication and interpersonal skills.
• Ability to work with families and referral sources.
• Attention to detail and data handling in EHR systems.

Responsibilities

• Attends mandatory staff meetings and other meetings as assigned.
• Receives and tracks incoming referrals.
• Gathers information from referral sources and documents it.
• Collaborates with families and team for scheduling.
• Maintains evaluator schedules for timely appointments.
• Manages data in the EHR system and ensures documentation accuracy.
