Job summary

Friends Outside in California seeks a Senior Social Worker to serve as probation navigator for Tuolumne County residents. The role conducts screenings, coordinates diversion referrals, and connects clients to community-based services supporting long-term stability.

You will collaborate with probation, behavioral health, and other partners, maintaining a trauma-informed, client-centered approach while meeting grant requirements and reporting outcomes.

Qualifications

• Master’s degree in social work.
• Minimum of three years of professional experience in social services or related fields.
• Experience collaborating with courts, probation, and community organizations.
• Experience with grant-funded programs and outcome reporting preferred.

Responsibilities

• Serve as the primary liaison between Probation and community-based service providers.
• Facilitate warm handoffs and introduce clients to service providers to improve engagement.
• Provide resource information and ongoing support to youth and families through referrals.
• Monitor service participation and address barriers to engagement.
• Assist with identifying and obtaining essential documents (ID, etc.).
• Connect clients to programs like Medi-Cal, CalFresh, and other public benefits.
• Coordinate appointments, reminders, transportation, and engagement support.
• Support compliance with court and program requirements.
