Signature Healthcare at Home Hospice in Portland, OR is seeking a compassionate Registered Nurse (RN) to join as a PRN Weekend Admissions Nurse. You will respond to referrals, perform admission assessments, and coordinate initial care plans for patients and families during transitions to hospice services.

Qualifications

• Current Oregon RN license in good standing.
• Minimum one year of nursing experience.
• Valid driver's license, reliable transportation, and proof of auto insurance.
• Strong assessment, communication, and organizational skills.
• Ability to work independently and efficiently in the field.

Responsibilities

• Respond to hospice referrals and admission requests during weekend coverage hours.
• Complete comprehensive hospice admission assessments in patients' homes or care settings.
• Evaluate patient eligibility for hospice services under Medicare guidelines.
• Coordinate admission paperwork, consents, and required clinical documentation.
• Develop and initiate individualized plans of care.
• Provide education regarding hospice philosophy, services, medications, symptoms and resources.
• Collaborate with physicians, nurses, social workers, chaplains, aides and other team members.
• Communicate effectively with referral sources, hospitals, and community partners.
• Ensure timely and accurate completion of documentation in the EMR.
• Support patients and families with compassion, dignity and professionalism during transitions.
