Adventist Health Portland is seeking a Registered Nurse for our home health and hospice teams serving the Portland area. The role plans, organizes and directs home care services and manages a patient caseload with collaboration across disciplines.

Responsibilities include completing comprehensive nursing assessments, implementing and adjusting plans of care, thorough documentation, patient and family education, and ensuring compliance with HIPAA and AH policies.

Qualifikationen

• RN licensure in the state of practice (Required)
• CPR certification or Basic Life Support (BLS) certification (Required)
• Valid Driver's License and must be at least 21 years of age (Required)
• Current automobile insurance and a reliable automobile (Required)

Aufgaben

• Performs initial and ongoing nursing assessments of the patient’s needs.
• Implements plan of care and collaborates with patient and family to set goals.
• Documents care delivery, patient status, response to treatment, and interventions.
• Counsels and educates patient and family; provides learning opportunities and preventive procedures.
• Complies with HIPAA, code of conduct and documentation standards; on-call duties may be assigned.
• Performs other job-related duties as assigned.
