Job summary

Connected Home Health & Hospice in the Portland metropolitan area is seeking a full-time, hourly Admissions RN to lead SOC assessments, start care plans, and establish a solid foundation for each home-health episode.

This role follows normal business hours, Monday through Friday, with strong interdisciplinary support and a balanced workload, offering predictable scheduling, patient-centered care, and opportunities for professional growth within The Pennant Group.

Qualifications

• Current Oregon RN license or ability to obtain.
• One year of nursing experience preferred; home-health or admissions experience a plus.
• Strong assessment, communication, and critical-thinking skills.
• Ability to work independently in a field-based role.
• Reliable transportation and valid driver’s license.

Responsibilities

• Complete SOC nursing assessments in patient homes.
• Initiate individualized care plans and establish expectations for the home-health episode.
• Conduct medication reconciliation and safety assessments.
• Coordinate care with interdisciplinary team to start services promptly.
• Provide skilled nursing interventions during SOC visits.
• Maintain accurate documentation per regulatory and organizational standards.
• Communicate effectively with physicians, caregivers, and internal team members.
• Support safe transitions from hospitals, SNFs, and outpatient settings.
