Job summary

St. Peter's Health Partners is seeking a compassionate Registered Nurse to join the Weekend Hospice Admissions Team. The RN will evaluate and admit hospice patients in home settings, with cross-training to other hospice care levels. Work primarily Saturdays and Sundays, 8a-8p, with training ensuring coverage and safety protocols.

The role offers full-time medical benefits and a supportive environment with supervisory guidance, training, and field support for efficient patient handoffs.

Qualifications

• Valid NYS RN license
• Associate’s degree in nursing (BSN preferred)
• Minimum 1 year RN experience, home health experience preferred
• Valid driver license and reliable vehicle
• Strong communication, documentation and time management skills
• Proficient with computers and electronic charting

Responsibilities

• Conduct hospice admissions for patients in their homes, averaging two home admissions per day.
• Collaborate with community physicians and Hospice Medical Directors to confirm hospice eligibility.
• Complete comprehensive assessments of patient and family needs.
• Initiate the plan of care, including coordination of medications, equipment and services.
• Coordinate care with social workers, chaplains, volunteers and other team members.
• Ensure a seamless handoff to the Hospice Case Manager and accurately document related information.
