Job summary

Avera is seeking a Speech-Language Pathologist to evaluate patients, develop treatment plans, and provide therapy in home health settings. The role requires ASHA certified or eligible, and state licensure upon hire, with 1–3 years of hospital/home health experience preferred.

The PRN position offers working hours as needed, within a multidisciplinary team focused on improving patient living and communication outcomes. Collaboration with nursing and rehabilitation teams is essential.

Qualifications

• CCC-SLP certification from ASHA or active supervised fellowship in progress.
• State licensure as Speech-Language Pathologist required upon hire.
• 1–3 years of hospital, SNF, outpatient or home health experience preferred.

Responsibilities

• Evaluates patients through assessment, interprets results, and plans treatment.
• Develops individualized treatment programs addressing speech, language, cognition, voice and dysphagia.
• Provides direct therapy in home health and coordinated settings.
• Communicates with patients, families and medical team about progress and goals.
• Maintains documentation of evaluations, plans, progress notes and reports.
