Avera Orthopedic Hospital-Sioux Falls is seeking a PRN Speech-Language Pathologist to evaluate patients, develop treatment plans, and deliver therapy addressing speech, language, cognition, voice, and dysphagia in the home setting when needed. The role requires collaboration with a multidisciplinary team and adherence to CARE standards.

Responsibilities include using assessments to guide intervention, planning goals, documenting results, and coordinating with physicians and families.

Qualifications

• CCC-SLP certification or supervised fellowship in progress with ASHA.
• Active license to practice Speech-Language Pathology in the relevant state.
• Experience in hospital, SNF, outpatient or home health is preferred.

Responsabilités

• Evaluates assigned patients through formal and informal assessments to determine needs.
• Assesses age-specific factors from infants to geriatrics per guidelines.
• Develops and implements individualized treatment programs.
• Sets intervention goals and adapts plans based on ongoing evaluations.
• Provides direct therapy services in various settings to address speech needs.
• Plans therapy considering the patient’s physical, emotional, and cognitive abilities.
• Prepares evaluation results, treatment plans, progress notes, and reports.
• Communicates with patient, family, physician, and care team about status and needs.
• Collaborates with interdisciplinary team and families throughout intervention.
