Job summary

Qualifications

• Graduated from an accredited program approved by their professional association.
• Current licensure by the applicable state agency.
• Clinical experience in a skilled nursing facility is preferred.

Responsibilities

• Evaluates residents and develops appropriate care plans.
• Under a physician's order, provides direct resident treatment per plan of care.
• Documents services provided within Rehab Optima on a timely basis.
• Demonstrates knowledge of Medicare and other reimbursement regulations.
• Participates in training of staff and quality improvement activities.
