Job summary

Pocopson Home in West Chester, PA, is seeking a PRN Speech-Language Pathologist to join our rehab team. Work with adults and geriatric patients in a skilled nursing setting, with flexible weekday and weekend hours and a collaborative, therapist-led environment.

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• Current Pennsylvania SLP license or eligibility for licensure.
• CCC-SLP preferred.
• Experience with adult/geriatric populations, skilled nursing, dysphagia or cognitive-communication treatment is a plus.
• Strong communication and documentation skills.
• Collaborative, patient-first approach to care.

Responsibilities

• Speech, language and cognitive-communication evaluations and treatment.
• Dysphagia and swallowing assessment and intervention.
• Development of individualized, functional treatment plans.
• Patient, family and caregiver education.
• Collaboration with PT, OT, nursing and other members of the interdisciplinary team.
• Documentation of evaluations, treatments, progress and discharge recommendations.
