Job summary

H2 Health is seeking a Speech-Language Pathologist for Prime Living 1:1 patient care in senior living communities. This PRN role involves 4–10 visits per week, evaluating and treating a geriatric caseload with dysphagia, cognitive impairment, and neurologic disorders, delivering independent and collaborative care.

Candidates must hold CCC-SLP and active state license, with outpatient experience preferred. Flexible schedule, career growth, and access to in-house education are offered.

Qualifications

• Education: A graduate of an accredited Speech Language Pathology program.
• Experience: Prior experience in outpatient therapy is a plus. Experience with dysphagia is required.

Responsibilities

• Provide 1:1 patient care in geriatric populations.
• Work independently and within interdisciplinary teams to tailor therapy plans.
• Evaluate and treat dysphagia and cognitive/neurologic conditions.
• Ensure appropriate decisions based on each patient’s functional needs.
