Job summary

AdvantageCare Rehabilitation, Pocopson Home in West Chester, PA, is seeking a PRN Speech-Language Pathologist (SLP) to join our therapy team in a skilled nursing and rehabilitation setting. The role offers weekday and weekend PRN coverage with flexible scheduling to accommodate other positions while supporting adult and geriatric residents.

You’ll work with an experienced, collaborative rehab team, focusing on dysphagia, cognition, and communication needs.

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• Current Pennsylvania Speech-Language Pathologist license or eligibility for licensure.
• CCC-SLP preferred.
• Experience with adult/geriatric populations, skilled nursing, dysphagia or cognitive-communication treatment is a plus.
• Strong communication and documentation skills.
• Collaborative, patient-first approach to care.

Responsibilities

• Perform speech, language, and cognitive-communication evaluations and treatment for adult/geriatric residents.
• Assess and treat dysphagia and swallowing disorders.
• Develop individualized, functional treatment plans and document progress.
• Educate patients, families, and caregivers; collaborate with interdisciplinary team.
• Maintain accurate evaluations, treatments, progress notes, and discharge planning.
