The position emphasizes coordinating referrals and follow-ups to bridge clinic and external providers. You will translate complex physician orders into clear education plans for patients and meticulously document progress to ensure seamless, long-term care outcomes.

Competenze

• Registered Nurse credentialed from the Indiana Board of Nursing before hire or transfer date.
• BLS Provider within 30 days of hire, American Heart Association or American Red Cross accepted.
• Instructor specializing in Basic Life Support within 30 days of hire, American Heart Association or American Red Cross accepted.
