Job summary
Village Caregiving is seeking a PRN Registered Nurse to join our home care team in Rhinelander, WI. You will visit patients in their homes, perform health assessments, document medical history, and develop personalized care plans for caregivers.
The role offers a flexible schedule and the opportunity to work independently while making a meaningful difference in the home care setting. Travel to client homes is required with support from the team.
Qualifications
- Graduate of an Accredited Diploma, Associate, or Bachelor’s School of Nursing, or Vocational Nursing.
- Current State RN License.
- CPR certification.
- Compassionate, friendly demeanor.
- Excellent communication skills.
- Driver’s license and reliable transportation.
- Ability to work a flexible schedule and travel.
Responsibilities
- Visit patients in their homes for one-on-one assessments.
- Complete initial and ongoing assessments to identify needs.
- Develop care plans for caregivers to follow.
- Educate patients and families on home health strategies and devices.
- Maintain clear communication with patients, families, and health professionals.
- Complete nurse assessment documents on a tablet and upload to CRM.
- Coordinate schedule with Director for upcoming assessments.
