Job summary

Village Caregiving in Rhinelander, WI seeks a PRN Registered Nurse to join our home care team. You will visit clients for health assessments, document medical histories, and help develop personalized care plans for caregivers in the Rhinelander area. Travel is required.

The RN may assist training new caregivers and enjoys a flexible schedule, thriving in an independent role that makes a meaningful difference in home health care.

Qualifications

• Graduate of an Accredited Diploma, Associate, or Bachelor’s School of Nursing, or Vocational Nursing.
• Current State License As an RN.
• Proof of Current CPR Certification.
• Compassionate, Friendly Demeanor.
• Excellent Communication Skills.
• Current Driver’s License and Reliable Transportation.
• Ability to Work a Flexible Schedule and Travel.

Responsibilities

• Visit Patients in Their Homes for One-on-one Visits.
• Complete Initial and Ongoing Assessments to Identify the Physical and Environmental Needs of Patients.
• Assess Patients and Their Environment to Develop a Care Plan in Order for the Caregiver to Have a Blueprint to Know How to Safely and Competently Take Care of Our Patients.
• Educate Patients and Their Families on Proper Home Health Care Strategies and Procedures, and Make Recommendations for Devices or Tools That Might Improve the Patient’s Quality of Life.
• Provide Effective Communication to Patients/Clients, Their Family Members, Team Members, and Other Health Care Professionals.
• Fill out Nurse Assessment Documents on a Supplied Tablet the Same Day As Assessment. Upload Assessment to CRM.
• Keep Timely Communication with the Director on Schedule for Upcoming and Current Assessments.
