Village Caregiving is seeking a PRN Registered Nurse to join our home care team, serving clients in the Eagle River, WI area. You will visit patients at home to assess health status, document medical history, and develop personalized care plans for caregivers.

This PRN role offers a flexible schedule, travel to client homes, and the chance to work independently while making a difference in home-based care.

Qualifications

• Graduate of an accredited Diploma, Associate, or Bachelor’s School of Nursing, or Vocational Nursing.
• Current State License as an RN
• Proof of CPR certification
• Compassionate, friendly demeanor and excellent communication skills
• Driver’s license and reliable transportation
• Ability to work a flexible schedule and travel

Responsabilités

• Visit patients in their homes for one-on-one visits
• Complete initial and ongoing assessments to identify needs
• Develop care plans to guide caregivers on safety and care
• Educate patients and families on home health care strategies and devices
• Maintain clear communication with patients, families, and health professionals
• Complete nurse assessment documents and upload to CRM the same day
• Coordinate schedule with director for upcoming assessments
