Job summary

Elara Caring is seeking a Registered Nurse Home Health in Houston, TX to support our mission of delivering right care, at the right time, in the right place. You will assess patients, create and adjust care plans, and educate families while ensuring HIPAA privacy and safety standards.

Requirements include an active RN license, graduation from an approved nursing program, 1+ year of clinical nursing experience, and reliable transportation to travel between patient homes.

Qualifications

• Current State license as a Registered Nurse
• Graduate of an accredited nursing program (Certificate, Diploma, ASN or BSN)
• 1+ year of clinical nursing experience
• Home Health experience preferred
• Ability to lift up to 50 lbs and assist patient handling
• Valid driver’s license or reliable transportation for travel between patient homes

Responsibilities

• Assess patients to determine care needs and develop plans
• Complete initial and ongoing patient assessments
• Monitor patient conditions and report changes to the care team
• Educate patients and families on care and safety
• Provide nursing care per care plan and physician orders
• Ensure privacy and HIPAA compliance
• Assist with transfers and positioning as needed

As a Registered Nurse Home Health , you’ll contribute to our success in the following ways:

• Assessing patients to determine their care needs.
• Completing initial and ongoing patient assessments.
• Protecting patient and staff privacy in accordance with HIPAA.
• Providing nursing care according to the plan of care and physician orders.
• Monitoring patient conditions and reporting changes to the care team.
• Educating patients and families on their care and safety.
• Taking appropriate action when patient conditions change.
• Helping ensure quality, safe, and continuous patient care.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
