Job summary

Elara Caring is seeking a Registered Nurse Home Health to join our team in Ohio. You will assess patients, conduct initial and ongoing assessments, and provide nursing care according to the care plan and physician orders.

The role requires maintaining patient privacy and monitoring conditions, with education for patients and families on care and safety. While home health experience is preferred, it is not required; a valid RN license and a willingness to travel between patient homes are

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited nursing program (Certificate, Diploma, ADN or BSN).
• 1+ year of nursing experience in a clinical care setting.

As a Registered Nurse Home Health, you’ll contribute to our success in the following ways:

• Assessing patients to determine their care needs.
• Completing initial and ongoing patient assessments.

• Providing nursing care according to the plan of care and physician orders.
• Monitoring patient conditions and reporting changes to the care team.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
