Elara Caring is seeking a Registered Nurse Home Health to provide in-home nursing care and support. You will assess patients, follow care plans, educate families, and coordinate with the care team to ensure safety and quality in home settings.

Strong clinical background and a valid RN license are required; 1+ year of nursing experience is needed, with home health experience preferred. Travel to patient homes is expected.

Qualifikationen

• Graduate of an accredited nursing program (Certificate, Diploma, Associate or Baccalaureate).
• At least 1 year of clinical nursing experience.
• Home Health experience preferred but not required.
• Ability to lift up to 50 pounds and assist with transfers.
• Valid driver's license or reliable transportation for travel between homes.

As a Registered Nurse Home Health, you’ll contribute to our success in the following ways:

• Assessing patients to determine their care needs.
• Completing initial and ongoing patient assessments.

• Providing nursing care according to the plan of care and physician orders.
• Monitoring patient conditions and reporting changes to the care team.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
