Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current CPR certification.
• Valid driver''s license and access to a dependable vehicle or public transportation.
• Ability to function in home settings regardless of client demographics.

Responsibilities

• Conduct Long Term Care insurance assessments in patients'' homes.
• Collect demographics, physician information, medication lists, and evaluate functional independence.
• Complete cognitive screening and document findings in real time via a web-based system.
