Job summary

Qualifications

• Active RN license in the state of practice (unrestricted).
• Valid driver's license, valid vehicle insurance, and reliable transportation.
• Current CPR certification.
• Ability to work with diverse client populations in home care.

Responsibilities

• Complete Long Term Care insurance assessments including demographics, physician info, med list.
• Evaluate functional independence (ADL/IADL) and cognitive screening.
• Document via point-of-care system in real time on laptop or tablet.
• Travel to patient homes as required.
