Job summary

Select Specialty Hospital - Boardman is seeking a PRN RN Clinical Liaison to advocate for referred patients, coordinate pre-admission assessments, and facilitate transfers in a LTACH setting. You will collaborate with physicians, families, and clinical teams to ensure smooth pathways to post-ICU care, while building relationships with facilities and payers.

The role offers a $40/hr wage and a $5,000 sign-on bonus, with PRN scheduling to meet patient access needs and strong clinical

Qualifications

• Current and valid RN licensure.
• Preadmission process experience with rehabilitation or post-acute care preferred.
• Experience in health care sales or marketing is a plus.

Responsibilities

• Plan the continuum of care after ICU or post-surgical stay with clinicians and families.
• Counsel families after clinical assessments to support care decisions.
• Collaborate with case managers and physicians to remove barriers for pre-admission and transfers.
