Select Specialty Hospital, a division of Select Medical, is seeking a Clinical Liaison (RN/RT) to advocate for referred patients, coordinate pre-admission assessments, and remove barriers to access post-ICU care. You will collaborate with physicians, discharge planners and families to plan the continuum of care after ICU or surgical stays.

This full-time role offers a $70,000 to $95,000 annual salary plus uncapped monthly bonuses and a $10,000 sign-on.

Qualifications

• RN or RT license required.
• Experience in patient advocacy is a plus.
• Ability to coordinate care plans with families and clinicians.

Responsabilités

• Advocate for referred patients in pre-admission and coordinate care.
• Collaborate with physicians, discharge planners and families on the continuum of care.
• Facilitate patient transfers and address payer-related barriers for post-ICU care.
