Job summary

Select Specialty Hospital in the LTACH segment seeks a Clinical Liaison (RN / RT) to advocate for referred patients during pre-admission, coordinate with clinical teams, and work with families and physicians on care plans. You will help remove barriers to accessing post-ICU care and coordinate transfers.

We offer a competitive salary with uncapped monthly bonus incentives and extensive onboarding. Full-time Monday–Friday, 8–5, with a focus on building relationships across medical facilities

Qualifications

• RN or RT license required; other designations considered on review.
• Ability to perform clinical assessments of medically complex patients.
• Strong communication and relationship-building skills.

Responsibilities

• Advocate for referred patients in pre-admission by coordinating with clinical teams.
• Coordinate care plans with physicians and families to ensure appropriate post-ICU care.
• Facilitate patient transfers and remove payer-related/operational barriers for admission.
