Job summary

Select Specialty Hospital is seeking a Clinical Liaison to advocate for referred patients during pre-admission, coordinating with clinical teams and families to assess needs and plan care after ICU or post-surgical stays.

You will work with physicians, discharge planners, and hospital staff to remove barriers to access and ensure smooth transfers to our LTACH. A strong clinical background and relationship-building skills are essential.

Qualifications

• RN or RT license required; other designations reviewed.
• Ability to complete clinical assessments of medically complex patients.
• Collaborate with physicians and families to plan care.

Responsibilities

• Plan continuum of care after ICU or post-surgical stay with clinicians and families.
• Conduct clinical assessments of referred patients and counsel families on care decisions.
• Collaborate with case managers and physicians to facilitate pre-admission and transfer.
