MUHA invites a RN Case Manager to join the Florence, SC Case Management team as a PRN Staff Nurse. The role coordinates the interdisciplinary care plan from admission to discharge and includes discharge planning and liaison duties with patients, families, and insurers.

Qualified candidates typically hold a BSN or higher, an SC- or compact-state RN license, and have case management experience. A CMA certification is preferred but not required.

Qualifications

• Bachelor of Nursing degree preferred with minimum of one year of experience.
• RN license in South Carolina or a compact state is required.
• Case Management certification preferred.

Responsabilités

• Coordinate the interdisciplinary plan of care from admission to discharge.
• Develop discharge plans to meet ongoing patient needs and encourage compliance with medical advice.
• Serve as liaison between patient/family, healthcare staff, and insurers; may conduct utilization reviews.
